A Healthcare Power of Attorney (HCPOA), also known as a medical Power of Attorney or a healthcare proxy, is a legal document that allows you to appoint someone you trust to make medical decisions on your behalf if you become incapacitated and can't make these decisions yourself.
Key points about a healthcare POA:
- Appoints a Healthcare Agent: You choose a person (your agent) to make healthcare decisions for you when you can't. This could be due to illness, injury, or mental incapacity.
- Grants Decision-Making Authority: Your healthcare agent has the authority to make a wide range of healthcare decisions, including consenting to or refusing medical treatments, surgical procedures, and end-of-life care.
- Complements a Living Will: While a Living Will states your preferences for end-of-life care, a healthcare POA appoints someone to ensure those preferences are followed and to make other necessary medical decisions.

- Choose Your Healthcare Agent: Select someone you trust to make healthcare decisions that align with your wishes. This could be a family member, close friend, or anyone you trust to act in your best interests.
- Provide Your Agent's Information: Enter your chosen agent's full legal name and contact information into the Origin platform.
- Specify Any Limitations: If you want to limit your agent's decision-making authority in any way, you can specify these limitations.
- Sign and Notarize: Once you've reviewed your healthcare POA, you'll sign it electronically. Some states require notarization- go here for more information.
- Distribute Copies: Provide copies of your healthcare POA to your healthcare agent, backup agent, doctor, and anyone else who might need to be aware of it.
